Accident Statistics Register Template

An accident statistics register template in Microsoft Excel for summarising accidents, incidents and near misses and turning them into yearly totals, days lost and frequency rates. A Google Sheets version is included.

ISO 45001 asks you to monitor and evaluate your health and safety performance, and accident figures over several years are one of the clearest measures of it. The register holds brief details only, with no personal data, alongside the full accident report kept elsewhere.

What's Included in this Accident Statistics Register

The workbook is laid out as separate sheets, each doing one part of the job:

  • Accident_Data - one row per accident, incident or near miss, with a number, the date, type, category, a short summary, whether there was an injury or lost time, total days lost and notes.
  • Summary - year by year totals by type, with total staff, hours worked, days lost, total accidents and lost time incidents, and the All Accident Frequency Rate and two lost time incident rates.
  • Analysis - charts of accidents and near misses by year and by category, days lost by year and the frequency and lost time rates, over six years from the start year you choose.
  • Instructions - how to use the register and how the rates are worked out, the lists behind the drop-downs, from fatality and major injury to minor first aid injury, dangerous occurrence and near miss, an annual hours worked calculator and the rate multipliers.

Frequency Rates Explained

The All Accident Frequency Rate is the number of injuries in a year divided by the hours worked that year, multiplied by one million. The lost time incident rate divides lost time injuries by hours worked and multiplies by 200,000, which represents 100 employees working 50 weeks a year. Enter the staff numbers and hours for each year on the summary sheet and the register works both out.

Setting Up the Register

Enter the categories that suit your work on the Instructions sheet, use the hours worked calculator to work out total hours for each year, and set the start year for the analysis. The register comes with example accidents marked as test data. Cells with a red triangle in the corner carry help text.

Built for Health and Safety Reporting

  • Year by year figures ready for management review and health and safety objectives
  • Lost time and days lost tracked alongside the number of accidents
  • Near misses and dangerous occurrences counted with accidents, so the full picture is shown
  • No personal details, so the figures can be shared

Who Needs an Accident Statistics Register?

Any organisation that records accidents and wants the figures to show trends over time, whether for ISO 45001, for health and safety objectives or for the accident statistics asked for in prequalification and accreditation questionnaires.

Pairs with the Accident Report Form

Each accident is reported in full on the F-HS13 Accident Report Form, with just a summary carried into this register. PP-7-04 Accident and Incident Reporting Policy Procedure sets out how accidents are reported and investigated, and GG-7-04 Accident and Incident Reporting gives staff guidance on what to report.
